PRE-QUALIFICATION REQUIREMENTS

To be eligible for placement with an assistance dog, an individual must:

• Have a diagnosed physical disability, debilitating chronic illness or neurological
disorder.

• Be physically and cognitively capable of participating in the training process and
follow up independently or with assistance.

• Be able to independently command and handle the assistance dog.

• Commit to meeting the emotional, physical and financial needs of the assistance dog.

• Live in a stable home environment.

• Have no other pet dogs in the home (can have other domestic animals).

• Involvement with the organization before, during and after placement with an assistance dog; in regards to participation with the acceptance process, pre-matching training and annual re-certification after placement.

• Live in an area serviced by a Topaz: within the state of Texas.

SEIZURE APPLICATION REQUIREMENTS

• Minimum one year under the care of a Neurologist for seizure control prior to requesting an application.

• Minimum one year under the care of a Neurologist for seizure control prior to requesting an application.

• Individual must have a minimum of two epileptic seizures per month.

• T.C.S. does not train dogs to predict oncoming seizure activity or to break falls.

AUTISM APPLICANT REQUIREMENTS

• Have a medical diagnosis on the autism spectrum.

• Children must be between the ages of 3 and 12 years old at the time of requesting an application.

• The parent or guardian must be the primary handler of the assistance dog. T.C.S. does not permit autism assistance dogs to accompany the child to school alone.

MOBILITY APPLICANT REQUIREMENTS

• Have a diagnosed debilitating physical or neurological disability that effects movement or mobility.

• For the health and safety of our dogs T.C.S. does not train or permit dogs to provide long-distance downward pressure bracing support.

APPLICATION PROCESS

• Submit Preliminary Application: If the preliminary application meets our placement criteria, you will be contacted to submit a full application within 14 business days.

• Submit Full Disability Application along with completed medical reference forms from treating Doctor within 30 days along with a non-refundable $75 application fee for consideration.

• Background check clearance for applicant and residents in home.

• Conduct in-home needs assessment interview.

Submitting the application and completing the interview process does not automatically guarantee acceptance for an assistance dog. Upon completion and review of the entire application process acceptance will be determined. Topaz Canine Services reserves the right to deny an applicant at any stage of the process in acquiring a T.C.S. assistance dog.

PLACEMENT PROCESS

Upon being accepted for partnership with an assistance dog, Clients will:
• Attend scheduled training meetings as applicable to improve dog-handling skills and knowledge about life with an assistance dog before being matched.

• Meet your new partner!

• Team Training: Two weeks working hands-on alongside our Trainers in Dallas to learn how to work and navigate life with your assistance dog.

• Certification: Skills Assessment Test and Public Access Test

• Monthly follow up checks for the first year and lifetime ongoing support.

• Dog ownership transferred after one year of successful partnership.

• Annual re-certification testing.

Clients are financially responsible for travel and accommodations during the Team Training period. At this time Topaz is unable to provide transportation or lodging assistance.
